How SMS consent is recorded
The Get Help form uses a separate SMS authorization checkbox. When selected, the submission records the consent-text version, submission page, timestamp, and requested channel. Providing a phone number alone does not authorize SMS.
You must provide a mobile number you control and select text message as an authorized contact channel. Email permission is not copied into SMS permission, and SMS permission is not copied into ongoing marketing-email permission.
Types of messages
Messages may concern the request you submitted, scheduling, documents needed for a review, enrollment or servicing steps, deadlines, or follow-up you requested. Lakeland Health Insurance does not guarantee a minimum or maximum message count.
Do not send Social Security numbers, Medicare numbers, payment-card information, medical records, policy passwords, or carrier-portal credentials by SMS. David may redirect sensitive matters to an appropriate secure channel.
Opt-out and withdrawal
Reply STOP to withdraw SMS consent. An opt-out applies prospectively and may not erase records that must be retained for compliance or an existing transaction. After opting out, use phone or email if you still want assistance through another channel.
If a mobile number changes ownership, the new owner should reply STOP or contact David. You are responsible for keeping the number associated with your request current.
Privacy, sharing, and retention
Mobile information and SMS consent are not sold or shared with third parties for their own marketing purposes. Communications providers may process the number, message content, technical delivery data, and opt-out status to operate the messaging service.
Consent and message records are retained only as long as reasonably necessary for service, security, and applicable insurance, Marketplace, Medicare, carrier, legal, and compliance obligations. Retention varies by record type and governing requirement.
