We collect information needed to respond to your request, compare coverage, support enrollment, service policies, and meet legal obligations.
We do not sell personal information to data brokers or unrelated marketers. Service providers may process data for defined business purposes.
Phone, email, and SMS preferences are handled separately. SMS consent is not a condition of purchasing insurance and is not shared for third-party marketing.
Lakeland Health Insurance is the public-facing business name used by David Huff, a licensed Florida insurance broker (FL License #W371813; NPN 18213932), through HealthMarkets. Lakeland Health Insurance is not an insurance carrier, a government agency, or the federal Health Insurance Marketplace.
We help individuals, families, and small businesses evaluate insurance options, including ACA Marketplace, Medicare Advantage, Medicare Part D, Medicare Supplement, fixed-indemnity, dental, vision, life, and other ancillary insurance products.
This policy applies to information handled through this website, requested phone or video conversations, text messages, email, in-person meetings, enrollment support, and third-party platforms used to provide those services. David provides local assistance in Lakeland and Polk County and remote assistance across Florida. Product and plan availability still depends on ZIP code, county, eligibility, appointment or certification where applicable, and plan year.
Important distinction: General website inquiries are not secure enrollment channels. Do not send a Social Security number, Medicare ID, financial account number, or detailed medical records through a general contact form, ordinary email, or text message unless you are specifically directed to an approved secure workflow.
We do not offer every plan available in your area. Currently we represent organizations that offer products in your area. Please contact Medicare.gov, 1-800-MEDICARE (1-800-633-4227), or your local State Health Insurance Assistance Program (SHIP) to get information on all of your options.
For scheduled personal marketing appointments involving Medicare Advantage, Medicare prescription drug, or cost-plan products, CMS generally requires a documented Scope of Appointment at least 48 hours in advance. CMS rules include exceptions for inbound calls and certain unexpected attendees. When a Scope of Appointment is required, it identifies the product types authorized for discussion and does not obligate you to enroll.
The information collected depends on how you interact with us and which service you request.
| Contact and identity | Name, address, email address, telephone number, date of birth, household details, and identity-verification information. |
|---|---|
| Coverage and eligibility | Current coverage, requested coverage, ZIP code or county, household size, tax-filing information, estimated income, and qualifying-life-event details. |
| Health and enrollment | Doctors, prescriptions, medical needs, policy or member identifiers, and other information needed for plan comparison or enrollment. Sensitive identifiers are requested only when necessary and through an appropriate workflow. |
| Communications | Messages, call details, appointment information, communication preferences, consent records, and recordings of covered Medicare marketing, sales, or enrollment calls. |
| Website activity | IP address, approximate location, browser and device information, referring page, pages viewed, interactions, and campaign or conversion data collected through analytics and advertising technologies. |
When authorized and relevant to the requested service, we may receive information from insurance carriers, HealthMarkets, CMS systems, HealthCare.gov or Marketplace systems, communication providers, scheduling tools, and other service providers. We use that information only for the applicable business, service, compliance, or enrollment purpose.
We may use information to:
When you request a call or separately agree to telephone contact, we may call about that request or the insurance products and services covered by your permission. Consent to marketing contact is not a condition of purchasing an insurance product. You may ask us not to call, and channel-specific consent may be revoked using the contact methods below.
CMS requires TPMOs to record Medicare marketing, sales, and enrollment calls, including the audio portion of web-based calls, in their entirety. Calls that do not involve marketing, sales, or enrollment are not subject to that CMS recording requirement. A recording disclosure is provided when applicable. Covered recordings are retained as required by CMS, the applicable plan sponsor, or other governing requirements.
If you separately opt in to SMS, messages may concern an insurance request, appointment reminders, policy information, or other subjects described at opt-in. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for assistance. SMS consent is not a condition of purchase.
Mobile information is not shared for third-party marketing: We do not sell, rent, or transfer a mobile number, SMS opt-in status, or text-messaging consent to third parties or affiliates for their own promotional or marketing purposes. Providers that help deliver or administer the SMS program may process mobile information only for those operational purposes.
Review the complete SMS Terms of Service and Privacy Policy for supported keywords, opt-out instructions, carrier disclaimers, and program-specific data practices.
Commercial email includes sender identification, a valid postal address, and an unsubscribe method as required by the CAN-SPAM Act. Requests to stop marketing email are processed within the legally required period. Transactional or relationship messages about an application, enrollment, policy, or requested service may continue when needed.
We use administrative, technical, and physical safeguards designed for the nature of the information handled. These measures include encrypted HTTPS transmission for this website, access restrictions, secure service providers and enrollment platforms, authentication controls where available, and procedures for handling sensitive records.
No transmission, platform, or storage system can be guaranteed completely secure. If a breach involving Florida residents triggers notice obligations under the Florida Information Protection Act, notice will be provided as expeditiously as practicable and within the period required by law, subject to legally permitted delays or exceptions.
Insurance and health-related information is protected under the laws and contractual requirements that apply to the specific organization, transaction, and data. This policy does not represent that every record handled by Lakeland Health Insurance is protected health information subject to HIPAA.
Information is kept only as long as reasonably necessary for the purposes described in this policy, including service, recordkeeping, dispute resolution, fraud prevention, contract, carrier, and legal requirements. Retention periods vary by record type:
When information no longer needs to be retained, it is deleted, de-identified, or disposed of using measures appropriate to the record and system.
Depending on where you live, the information involved, and applicable exemptions, you may have rights to request access, correction, deletion, or a copy of certain personal information; opt out of certain targeted advertising or data sharing; appeal a denied request; or use an authorized agent.
