How should Lakeland residents compare Medicare Part D drug plans?
Stand-alone Part D and drug coverage inside Medicare Advantage both change every January 1. Premium is only one line. Formulary, pharmacy, and Extra Help status usually move the yearly cost more.
Direct answer: Don't let your drug plan renew on autopilot. For 2027, Medicare caps covered drug costs at $2,400, but premiums, drug lists and pharmacy networks still change. Florida has 11 stand-alone drug plans, and none is $0 for people without Extra Help. Check every drug, dose and pharmacy on Medicare Plan Finder before December 7.

CMS’s September 28, 2026 press release said stand-alone Part D average premiums move from $35.09 in 2026 to $36 in 2027, and Annual Enrollment runs October 15 through December 7, 2026. The Florida state sheet lists 11 stand-alone Part D plans, a lowest advertised premium of $5.30 a month, and that 52.69% of Florida stand-alone enrollees can find a lower premium than in 2026. About 23.50% of those Florida enrollees get Extra Help.
KFF reported on October 2, 2026 that there are no $0-premium stand-alone Part D plans for people without Extra Help in 2027, that some current-plan premiums rise $50 or more a month if the person does not switch, and that Florida is the only state gaining a stand-alone plan (10 to 11). Those are reasons to compare. They are not a ranking of any plan.
CMS set the 2027 Part D out-of-pocket threshold at $2,400 for covered drugs, up from $2,100 in 2026. Premiums, which drugs are covered, and which pharmacies are preferred still change plan by plan. CMS’s landscape fact sheets say Medicare health and drug plan 2027 Star Ratings will be available on Medicare.gov on or around October 8, 2026. Treat them as one input, not the decision.
Stand-alone drug plan or drug coverage inside Medicare Advantage?
Standalone Part D with Original Medicare / Medigap
If you have Original Medicare, you join a stand-alone Part D plan for drug coverage unless you have other creditable drug coverage. Medigap does not replace Part D. You can keep or change that stand-alone plan during Annual Enrollment.
Drug coverage inside Medicare Advantage
Most Medicare Advantage plans include Part D. You still verify the formulary, tiers, prior authorization, quantity limits, and pharmacy network for the exact 2027 plan. Switching from a stand-alone Part D plan to a Medicare Advantage plan (or the reverse) is a medical-coverage decision as well as a drug decision. The $2,400 out-of-pocket cap: what it does and doesn’t cover
The $2,400 figure is a cap on covered Part D drug costs for 2027. It is not a cap on the monthly premium. It does not make every drug cheap before you reach the cap, and it does not apply to drugs the plan does not cover. Confirm excluded drugs, insulin rules, and what counts toward the threshold in the plan documents and on Medicare.gov.
How to check each drug: tier, prior authorization, quantity limits
Use Medicare Plan Finder or the plan’s 2027 formulary. For each medication, confirm the drug name, strength, and form (tablet, capsule, inhaler, injection). Then check:
Whether it is on the formulary, and on which tier.
Prior authorization, step therapy, or quantity limits.
Two pharmacies a mile apart can price the same drug differently on the same plan. Preferred pharmacies usually cost less than standard network pharmacies. Mail-order and specialty pharmacies are separate checks. Look up the exact store name and ZIP for Lakeland, Winter Haven, or elsewhere in Polk on the 2027 plan. If your pharmacy is out of network, ask whether a 30-day transition fill exists and how long it lasts. Do not assume last year’s preferred pharmacy stays preferred.
Late-enrollment penalty and creditable coverage
Medicare.gov says that as long as you have creditable prescription drug coverage, you can wait to join a Medicare drug plan or a Medicare Advantage plan with drug coverage. You will not pay the Part D late enrollment penalty as long as you do not go more than 63 days without creditable drug coverage. The plan that provides the coverage has to tell you, and it sends that information every year. Keep the letter.

Extra Help changes the math
Extra Help can reduce or eliminate the Part D premium and deductible and lower what you pay at the pharmacy. CMS notes that people in a Medicare Savings Program get Extra Help automatically. Without Extra Help, KFF’s 2027 finding applies: there is no $0 stand-alone drug plan. Medicare.gov also says people with Extra Help or Medicaid may change drug coverage once per calendar month.

AEP and other windows in Florida
Annual Enrollment for 2027 coverage runs October 15 through December 7, 2026. A change made in that window generally starts January 1. Outside that window, you need a Special Enrollment Period. Common examples include Extra Help or Medicaid (once per calendar month), moving out of the plan’s service area, or a plan that is not renewing. See when you can switch Medicare plans in Florida. Do I need a new Part D plan if my premium went up?
Not automatically. A higher premium is a reason to compare, not a reason to stay or leave by itself. KFF reported that some 2027 stand-alone drug plan premiums rise $50 or more a month if the enrollee does not switch. Check every drug, dose, and pharmacy on Medicare Plan Finder, then compare total yearly cost.
Does the $2,400 cap include my premium?
No. CMS set the 2027 Part D out-of-pocket threshold at $2,400 for covered drug costs. The monthly plan premium is separate and does not count toward that cap. Confirm the current rule on Medicare.gov and in the plan’s Evidence of Coverage.
What if my pharmacy isn't preferred on the new plan?
You can still fill many prescriptions at a standard network pharmacy, but the cost is often higher than at a preferred pharmacy. Mail-order and specialty pharmacies follow separate rules. Check your exact pharmacy name and ZIP on the 2027 plan before you enroll.
What is the Part D late-enrollment penalty?
Medicare.gov says you can wait to join a Medicare drug plan if you have creditable prescription drug coverage, and you will not pay the late enrollment penalty as long as you do not go more than 63 days without creditable coverage. The penalty is added to your premium and can last as long as you have Part D. Confirm the current calculation on Medicare.gov.
Can I change drug plans after December 7?
Annual Enrollment ends December 7. After that, you need a Special Enrollment Period. Medicare.gov says people with Extra Help or Medicaid may change drug coverage once per calendar month. Other events, such as a move or a plan that is not renewing, have their own windows. Confirm the event that applies to you on Medicare.gov.
Will my drug cost less on Medicare in 2027, and does that change which plan I pick this fall?
Starting January 1, 2027, Medicare’s negotiated prices take effect for 15 more Part D drugs, including Ozempic/Wegovy, Trelegy, Xtandi and Janumet. A Milliman analysis estimates about 3% of non-EGWP Part D enrollees (people not in employer group Part D plans) could see lower 2027 out-of-pocket costs from the negotiated prices, and actual savers may be fewer. Plan choice still depends on each plan’s formulary and pharmacy network, so compare during Annual Enrollment.
