Frequently asked questions
Is Premier 5000 ACA-compliant?
No. Premier 5000 is fixed-indemnity supplemental insurance, not ACA major medical. Most Floridians pair it with an ACA plan or use it during a coverage gap.
Does Premier 5000 cover pre-existing conditions?
A 12-month lookback applies. Conditions you received care for in the 12 months before the policy effective date are generally not covered during the first 12 months of the policy.
Why is Premier 3 not available in Florida?
Premier 3 is not filed for sale in Florida. UnitedHealthcare's Florida Premier offering is Premier 5000, which has different benefit limits (notably $5,000/day hospital confinement and 20 Rx fills in Year 1).
How does Premier 5000 pay claims?
Premier 5000 pays fixed-dollar benefits per covered service, paid directly to you (or assigned to the provider). Benefits are not reduced by what other coverage pays.
